During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site code compliance inspection was initiated on 7/3/2025 and completed on 7/7/2025.. The on-site inspection began at 2:05 pm and ended at 4:35 pm. The inspector reviewed compliance in the areas listed above. There were 89 children present with 14 staff. The inspector reviewed 8 children?s records and 10 staff records on-site on 7/3/2025. This inspection included document review, tour of the facility, interviews, observations and measurements

Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.031-A-4
Each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.
Staff #3, who began employment over 10 weeks ago and was supervising a group of children during the inspection, did not have current certification by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.
Plan of Correction: Both completed.
Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the child abuse and neglect registry for one state and did not have results of a search of the sex offender registry from one state for Staff #5, who has been employed for more than 2 weeks and was working with children at the time of the inspection.
