Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on Tuesday, April 27, 2021 and concluded on Thursday, April 29, 2021. The supervisor was contacted by telephone to initiate the inspection. There were 30 children present and 4 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on review of staff record, and interview, the center did not ensure to obtain results of a sex offender registry check from any state in which the individual has resided in the preceding five years prior to first day of employment.

Evidence: In Staff#1's record (date of hire: 11/23/2020), there were no results of a sex offender registry search found. Interview with staff confirmed there was no documentation available during this inspection.
Plan of Correction: Youth Programs will ensure the state registry check is completed for this staff and is completed for all other staff prior to their first day.
