Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on April 9, 2026, as a part of the licensure period. The on-site inspection began at 9:50am and ended at 11:00am. The inspector reviewed compliance in the areas listed above. There were 55 children present and 13 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

The record for Child #3, enrolled for one month, did not contain documentation of current immunizations.
Plan of Correction: The facility will obtain Child #3's immunizations from the parent.
Standard 8VAC20-780-140-A
Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance.

The record for Child #3, enrolled for one month, did not contain documentation of a physical examination.
Plan of Correction: The facility will obtain the physical examination for Child #3 and document the record.
Standard 8VAC20-780-240-A
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.

Staff #4, employed seven months, did not complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment. The department sponsored orientation course was completed five months after the first day of employment.
Plan of Correction: Moving forward, the facility will ensure that staff complete the department's sponsored orientation course within 90 days of employment.
