During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Violations
4Staff record #3-7 did not have the date of employment or volunteering listed.
Each staff shall submit documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with the children.
Staff #1, employed 2 years; staff #2, employed one year; staff #3, employed six years; staff #5, employed one month; staff #6, employed one month; staff #7, employed two years; had no documentation.
Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.
Staff #3 & 4, who is working with children, has not completed this course which is overdue by two years.
Staff #1obtained seven hours of the 16 hours required in 2024. Staff #2,4 and 5, obtained no hours of training in 2024.
