Inspector Notes
An unannounced inspection was initiated along with Licensing Inspector Sharon Allen on 10/03/23 in response to the center self-reporting an injury to a child that required outside medical care while under the supervision of the center. Interviews were conducted; equipment was inspected; two staff records and one child record were reviewed. One violation was cited related to information submitted by the licensee. Based on interviews, staff did not ensure the care, protection, and guidance of children in care.
Evidence: On 09/19/23 at approximately 10:15 a.m. Child #1, 9 months old, fell out of a stroller hitting their head on the pavement. Child #1 was seen in the ER and had a bump and a bruise on the forehead . An interview with staff revealed that they were in a hurry to get outside for a walk and was not sure that the protective belt had been securely engaged around Child #1.

Plan of Correction: We held meeting on 11/06/23. During the training we went over stroller usage and safety practices to include: checking twice before leaving and often during outings.
