Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review.

A monitoring inspection was conducted on 9/17/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 6 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.


Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-111-180-A
Based on record review, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
Adult 2's most current Tuberculosis screening was conducted on 5/23/2019.
Plan of Correction: Adult #1 already has a current TB on file.
Adult #2 TB test administered 9/21/21 reading of the result 9/23/21.
