Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.
A monitoring inspection was initiated on 8/2/2021 and concluded on 9/7/2021 in response to a self-reported incident by the facility regarding behavior guidance. The Director was contacted by telephone and a virtual inspection was conducted. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child record and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions, please contact me at shahana.green@doe.virginia.gov.
Violations
2Evidence:
1. On 7/28/2021 at approximately 9:30am in the Toddler classroom, Staff member A was observed reading a book to the children that had buttons that were different colors. Staff A was observed telling Child A to "push yellow Child A. Yellow." during circle time. Child A was observed reaching for the book, and Staff A stated "See, Child A doesn't know anything." while slowly pulling the book away and then proceeding to ask another child to press yellow.
Evidence:
1. Staff A (doh: 6/21/2021) did not have a central registry finding on file at the time of the inspection.
