Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview, and a virtual tour of the program.
A monitoring inspection was initiated on 2/22/2022 and concluded on 2/25/2022. The Director was contacted by telephone and a virtual inspection was conducted. There were 41 children present, ranging in ages from 2 years to 6 years, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions, please contact me at shahana.green@doe.virginia.gov.

Standard 8VAC20-770-60-B
Based on a review of staff records, all staff did not have a sworn statement or affirmation prior to employment.

Evidence:
1. Staff #1 (doh: 12/27/2021) did not have a sworn statement or affirmation on file during the inspection.
Plan of Correction: We will get it within the required 10 business days and send to the Licensing inspector/keep it on file.
Standard 8VAC20-770-60-C-2
Based on a review of staff records, all staff did not have a central registry finding within 30 days of employment.

Evidence:
1. Staff #1 (doh: 12/27/2021) did not have a central registry finding on file during the inspection.
Plan of Correction: We will get it within the required 10 business days and send to the Licensing inspector/keep it on file.
Standard 8VAC20-780-210-A
Based on a review of staff records, all program leaders did not meet one of the following required criteria.

Evidence:
1. Staff #2 (doh: 2/14/2022), who is a lead teacher, did not have proof of education/experience on file during the inspection.
Plan of Correction: We will obtain the proof of education/experience for the staff member and place in the staff record.
Standard 8VAC20-780-70
Based on a review of staff records, 2 out of 3 staff records did not contain documentation of 2 or more references.

1. Staff #1 (doh: 12/27/2021) did not have documentation of 2 of 2 required references during the inspection.
2. Staff #3 (doh: 2/14/2022) did not have documentation of 1 of 2 required references during the inspection.
