Inspector Notes

An unannounced Monitoring Inspection took place on 09/02/2022 between the hours of approximately9:05 a.m. and 12:00 p.m. There were 9 classrooms observed with a total of 135 children with 25 staff within the supervision guidelines. The children were observed reading, completing a phonics activity, coloring, tracing letters and other organized activities. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, not each staff member submitted documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.

Evidence: Staff #3?s (start date: 08/15/2022) tuberculosis screening, dated 09/03/2022, was completed after the time of employment.
Plan of Correction: The center will make sure all TB tests are submitted prior to starting moving forward.
