Inspector Notes
An unannounced Monitoring Inspection took place on May 20, 2024 between the hours of approximately 12:20 p.m. and 2:25 p.m. There were 5 classrooms observed with a total of 97 children with 13 staff within the supervision guidelines. The children were observed eating lunch (chicken sandwiches, green beans and fruit), napping, and other organized activities. Positive interaction between staff and children were observed. A complete inspection of the physical plant, fire drill log, medication and emergency procedures was conducted during this inspection. There were 5 staff records and 5 children's records reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
4Based on record review, the center did not ensure that documentation of a negative tuberculosis (TB) screening was completed within the last 30 calendar days of the date of employment.
Evidence: Staff #3?s (date of employment: 05/13/2024) TB screening was dated 11/08/2023.
Based on observation, hazardous substances were not kept in a locked place.
Evidence: Clorox wipes, bleach sanitizer spray, multi-surface cleaner, disinfectant spray, three air fresheners, and glass cleaner were in an unlocked cabinet in Classroom 5, accessible to children.
Based on observation, medication was not kept in a locked place.
Evidence: Two inhalers were in an unlocked cabinet in Classroom 5.
Evidence:
1. Child #1 (start date: 06/12/2023), Child #2 (start date: 06/13/2022) and Child #4?s (start date: 06/18/2020) records indicate food allergies.
2. The center did not have a written care plan for Child #1, Child #2 and Child #4.
