Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on 3/8/22 and concluded on 3/10/22. The provider was contacted by telephone and a virtual inspection was conducted. There were 8 children (28 points) present with 3 caregivers supervising. The children were observed playing outdoors, and infants were observed sleeping. Infant bottles were checked for labeling. There were no medications on site. 2 children's and 2 caregiver's records were reviewed.

Standard 22.1-289.035-B-4
Based on records review and staff interview, the family day home did not obtain results of an out of state sex offender search and criminal history search for an employee that has resided outside of Virginia within the last five years, prior to the first day of employment.
Evidence:
1. Caregiver # 1's first day of employment was 3/7/22.
2. Caregiver # 1's record indicated that she resided in Puerto Rico and Hawaii within the last 5 years.
3. Caregiver # 1 stated that she resided in Puerto Rico from September 2015 through September 2019. Caregiver # 1 also stated that she resided in Hawaii from September 2019 through February 2022, after which she moved to Virginia.
4. The sex offender search for Caregiver # 1 for Hawaii and Puerto Rico was not completed to this date.
5. The criminal history check for Caregiver # 1 for Puerto Rico was not completed to this date.
6. The provider stated that she was not aware that she needed to complete these searches for Caregiver # 1.
Plan of Correction: The proof of criminal background & sex offender results in Hawaii & Puerto Rico were completed. The central registry searches will be completed and a follow up will be completed with the inspector.
