During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site renewal inspection was conducted on 1/30/25. The on-site inspection began at 12:20pm and ended at 1:20pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 caregivers; the point total was 29. The inspector reviewed 6 children?s records and 3 caregiver/household member(s) records while on-site. The inspection included document review, tour of the home and interview. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/11/25. A POC submitted after this date will not appear on the public website.

The license and violation notice from the most recent inspection report shall be posted in a conspicuous place on the licensed premises.

The violation notice from the 7/12/24 inspection report were not posted in the home.
Plan of Correction: Posted new one and made copies in case they disappear again.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

There was no documentation of a physical exam on-file for Child #1 who began attending in November 2024.

Medication shall be kept in a locked place using a safe locking method that prevents access by children.

An over-the-counter pain reliever medication belonging to a child was found to be not locked and sitting on top of the copy machine located in 1 of 3 child activity areas of the home.
Plan of Correction: Keep medication locked in med. cabinet.

Annually, the provider shall obtain the parent's signed acknowledgement for having reviewed the child's emergency contact information.

1. There was no documentation of the parents annual review of emergency information since the Child #9 enrolled in September 2023.

2. There was no documentation of the parents annual review of emergency information for Child #10 since 1/19/24.

The family day home shall obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements once every 6 months for children under the age of 2 years old.

1. The most recent immunizations on-file for Child #9 were dated May 2024, when the child was 9 months old.

2. The most recent immunizations on-file for Child #10 were dated January 2024, when the child was 10 months old.

3. The most immunizations on-file for Child #11 were dated February 2024, when the child was 12 months old.
Plan of Correction: Requested from parents 1/31/25. Obtained updated shot record for Child #9, #10 and #11. Done 2/3/25.
