During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring/ focused inspection was conducted on 8/1/25. The on-site inspection began at 10:41am and ended at 12:35pm. The inspector reviewed compliance in the areas listed above. There were 8 children present and 3 caregivers; the point total was 23. The inspector reviewed 13 children?s records and 3 caregiver/household member(s) records while on-site. The inspection included document review, tour of the home and interviews.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/8/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-210-A
In addition to satisfactory completion of First Aid/CPR training, caregivers shall obtain a minimum of 16 hours of training annually in areas relevant to their job responsibilities.

0 of 16 hours of annual training was documented for caregivers #1, #2 and #3 between the period of 7/26/24-7/25/25.
Plan of Correction: Completed CPR and Mat Training this past wknd. The 16 hours of Training is scheduled for the next two Saturdays [sic]
Standard 8VAC20-800-240-A
Areas and furnishings of the family day home, shall be maintained in a safe condition.

An occupied crib had a cot, table and safety gate blocking access on the service side of the crib.
Plan of Correction: Toddler cots will be put away when there is a baby in the crib. Service side will be clear.
Standard 8VAC20-800-800-B
The provider shall review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review and update to the emergency plan.

The emergency plan was last reviewed by the provider on 12/13/23.

Repeat violation.
The family day home shall obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements every 6 months until the age of 2 years old.

The immunizations on-file for Child #5 were dated August 2023 when the child was 1 year old.
