Inspector Notes

A Subsidy Health and Safety Inspection (SHSI) was conducted on 12/14/2023 from 11:15am to 1:50pm. A licensing inspection was conducted on the same date as the SHSI inspection. The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements not covered in licensing standards were reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Standard 8VAC20-790-600-D-1
Based on record review and interviews, it was determined that the vendor did not ensure that all staff who work directly with children shall have within 90 days of the date of employment or 90 days from subsidy vendor approval current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Evidence:
1. The records for staff #1 (date of hire 8/15/23) , #3 (date of hire 10/5/21) and #4 (date of hire 8/31/23) did not contain documentation of current CPR certifications.
2. The Director confirmed the current CPR certifications were not in the staff record.

Based on record review and interviews, it was determined that the vendor did not ensure that all staff who work directly with children shall have within 90 days of the date of employment or 90 days from subsidy vendor approval current certification in first aid appropriate to the ages of children in care.
Evidence:
1. The records for staff #1 (date of hire 8/15/23) staff #3 (date of hire 10/5/21) and staff #4 (date of hire 8/31/23) did not contain documentation of current first aid certifications.
2. The Director confirmed the current first aid certifications were not in the staff record.

Based on record review and interviews, it was determined the vendor did not ensure that staff who work directly with children shall, annually attend at least 16 hours of training and staff development activities, to include the departments health and safety update course.
Evidence:
The records for staff #3, #4, and #5 did not include documentation that the health and safety update training course had been completed.
