Inspector Notes
An unannounced monitoring inspection was conducted on 12/14/23 from 11:15am to 1:50pm. At the time of the inspection there were 40 children in care and 7 staff members. Children were observed watching holiday movies and eating lunch. Records were reviewed for six children and five staff members. Medication, evacuation drills, emergency supplies and other required records and postings were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.
Violations
5Based on record review, it was determined the facility did not ensure that a sworn statement or affirmation is obtained prior to the first day of employment or beginning to serve as a volunteer.
1. The records for staff #1 (date of hire 8/15/23) and staff #2 (date of hire 8/21/2023) did not contain documentation that a sworn statement or affirmation was obtained.
Evidence:
There was no documentation of an updated TB screening for staff #3. The last TB screening was dated 11/21/2021.
1. The records for staff #2 (date of hire 8/21/23) and staff #4 (date of hire 8/31/2023) did not contain documentation that the Virginia Department of Education sponsored orientation course was completed.
2. Staff #5(Director) confirmed the training was not in the record.
Evidence:
1. In the classroom #2 there was a can of TNT foaming disinfectant cleanser in an unlocked cabinet.
2. In Classroom #1 there was a can of Lysol disinfectant spray and sanitizer on top of a cabinet.
3. Staff #5 (Director) confirmed the cleaning materials were not secured in locked cabinets.
1. The allergy lists in all three classrooms showed children?s names and their allergies in a manner that anyone coming into the classroom could see this confidential information.
