A Subsidy Health and Safety Inspection (SHSI) was conducted on 9/10/24. A licensing inspection was conducted on the same date as the SHSI inspection. The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements not covered in licensing standards were reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Repeat Violation
Based on record review and interviews, it was determined that the vendor did not ensure all staff who work directly with children shall have within 90 days of the date of employment, current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
1.The record for staff #1 (date of hire 8/24/23) did not contain documentation of current CPR certification.

Based on record review and interviews, it was determined the vendor did not ensure that staff who work directly with children shall, in addition to preservice and orientation training, annually attend at least 16 hours of training and staff development activities, to include the departments health and safety update course.
Evidence:
Staff #1 (date of hire 8/24/23), staff #3 (date of hire 8/21/23) and #4 (date of hire 10/5/21) had not completed the annual health and safety update training as required.
