The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a safe and operable condition. There was a hole, approximately 2 inches in diameter, at the base of the wall in the large bathroom.

Plan of Correction: The facility responded: The Co-Directors will ensure that a daily safety checklist is completed for the entire center. The hole in the wall was covered and sealed with putty on the same day by the maintenance man.

Hazardous substances must be in a locked place that prevents access by children. There was paint and liquid adhesives in an unlocked storage closet in the large bathroom.
Plan of Correction: The facility responded: The closet door was locked during the inspection. Staff will ensure the door is locked at all times.

Children under 10 years of age shall always be within actual sight and sound supervision of staff. During the inspection, Child #1, a three year old, was observed walking alone through the hallway unsupervised.
Plan of Correction: The facility responded: Staff will ensure active supervision at all times and position themselves so that they can always observe all children. All staff received a copy of Active Supervision handout.
