Inspector Notes

An unannounced Renewal inspection was conducted today. A total of 11 children (32pts)with 2 staff supervising. The Provider was not present for this inspection. The older children were seated at the table preparing for a morning project. The project consisted of drawing and coloring a worksheet. The children were observed taking turns sharing crayons with staff nearby to assist if needed. The younger children were in the adjoining program room where they free played with puppets and interacted with staff and demonstrated side by side play with each other. Observation of the crib room, bathroom and remaining areas of the home was made. Some topical ointments onsite. No medication needs at this time and no identified food allergies. Once the project was completed, the children took turns putting them in their assigned sliding drawer to take home. The children transitioned smoothly to floor activities while one staff was observed sanitizing the table to prepare for lunch. A total of 4 children's records and all staff records were reviewed. All background checks remain current from the June inspection along with First Aid/CPR certifications. This inspection occurred between 10am and 12pm. Thank you for your cooperation today. Standard 8VAC20-800-210-A
Based on staff record review, 16 hours of annual training was not obtained.
Evidence:
Staff #1 received only 2 hours of annual training in the past year. Staff #2 received only 8 hours of annual training during the past year.
Plan of Correction: Each staff member will have 16 hours of training completed.
Standard 8VAC20-800-240-A
Based on observation of the family day home, one area of the home was not maintained in a safe condition.
Evidence:
Wooden bookshelves that were approximately 3 feet in height were relocated to the main programming space and were not maintained to prevent a child from pulling the furniture down on themselves.
Plan of Correction: The bookshelves will either be anchored or removed.

Based on observation of the family day home, child resistant protective covers were not install on all unused electrical outlets.
Evidence:
An outlet located in the spare bedroom in the basement did not have protective covers.
Plan of Correction: All electrical outlets will be covered.

Repeat Violation
Based on review of the provider's documented evacuation drills, monthly drills were not completed.
Evidence:
The last evacuation drill occurred on 03/28/2023.
Plan of Correction: Monthly drills will be done this month and every month from now on.
Standard 8VAC20-800-830-B
Based on review of the provider's emergency drill documentation, shelter in place procedures were not practiced a minimum of twice a year.
Evidence:
The last documented shelter in place drill was dated in September 2022.
Plan of Correction: Shelter in place drills will take place every six month beginning this month.
