During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on February 10, 2025 and completed on February 10, 2025, as a part of the licensure. The on-site inspection began at 9:17am and ended at 10:30am. The inspector reviewed compliance in the areas listed above. There were twelve children present and three caregivers; the point total was 32. The inspector reviewed five children?s records and five caregiver/household member(s) records on-site. This inspection included document review and a tour of the family day home.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The provider must receive the results of the national fingerprint check prior to the caregiver's first day of employment.

Caregiver D's results of the national fingerprint check was received fifteen days after employment.
Plan of Correction: Will make sure to obtain the results of the national fingerprint check prior to hire in the future.

The provider must request a search of the central registry prior to the employee's first day of employment.

Caregiver D, who had been employed for three months, did not have a central registry search requested.
Plan of Correction: Will request and complete the central registry check. Will make sure to request prior to hire in the future.

REPEAT VIOLATION
Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment.

Caregiver B, who has been actively working at the center for two years and two months, did not have one of two completed reference verifications on file. Caregiver D, who has been actively working at the center for three months, did not have any completed reference verifications on file.
Plan of Correction: Will complete the missing reference checks and make sure to check references prior to hire in the future.
