Inspector Notes

A renewal inspection was initiated on 12/5/2022) and concluded on 12/5/2022. There were 55 children directly supervised by 10 staff members. Children were observed during lunch time, engaged in group activities, practicing a fire drill and outdoor play time. The physical plant, 3 child records, 3 staff records, medications, evacuation drills, emergency supplies and policies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-240-B
Based on a review of staff records, staff have not completed orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Evidence: staff #1, date of hire 9/26/2022 was observed supervising children, there is no documentation that she has completed orientation training.
Plan of Correction: Management will complete staff's orientation and document.
Standard 8VAC20-780-245-L
Based on staff record review and staff interview, there is not at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.
Evidence: There is currently no staff with Daily Health Observation, 2 of the staff's DHO expired in February 2022.
Plan of Correction: Staff will take the Daily health Observation training and file their certificates in the staff's record.
Standard 8VAC20-780-580-B
Based on observation and staff interview, the vehicles used to transport children does not meet the safety standards set up the Department of Motor Vehicles.
Evidence: Both of the center buses have expired State inspections, bus #1 expired 8/2022 and bus #2 expired 6/2022. Bus #2 has expired license plates, expired 8/2022.
Plan of Correction: Management will obtain the updated state inspections for both buses and obtain the updated tags for the license plate.
