Inspector Notes

An unannounced monitoring inspection was conducted on November 1, 2023, from 10:00am-1:00 pm. At the time of entrance, (81 ) children were in care with (15 ) staff members present. The inspector reviewed 5 children?s records and 5 staff records. Children were observed during snack time, engaged in individual activities, site words tracing and writing, difference between fruits and vegetables, cutting using scissors, farm animals and outdoor play time. Medications were observed. Areas of non-compliance are identified on the violation notice.

Standard 22.1-289.035-A
Based on a review of staff records, 1 staff has not obtained updated background checks every 5 years from the last checks.
Evidence: Staff # 1 hired 3/8/2017 and transferred to this center 9/4/2023 does not have updated fingerprints, sworn disclosure statement and central registry within 5 years from the last checks.
The fingerprints are dated 9/10/2018, sworn disclosure statement dated 9/18/2018 and central registry results dated 5/5/2017.
Plan of Correction: The staff will complete the sworn disclosure statement today and file for the central registry search and make an appointment to be fingerprinted today. The results will be filed in the staff's record.

Based on a review of staff records, staff has not obtained our of state background checks for 1 staff member.
Evidence: Staff # 5, hire date 9/11/2023 indicated on the sworn disclosure statement they have resided in Florida within the past 5 years. There is no documentation to show that a central registry search has been completed for the state of Florida.
Plan of Correction: Management will file the appropriate papers for the staff's Florida central registry search today. The results will be filed in the staff's record.

Based on a review of staff records, several staff have not completed the required recognizing child abuse and neglect training no later than 7 days of the date of assuming job responsibilities.
Evidence: The following staff were observed caring for children this date without taking the required child abuse and neglect training.
