Inspector Notes

An unannounced monitoring inspection was conducted on Ocotober 5, 2023 from 9:20am - 10:40am. There was two caregivers and five children present. Children were observed freely playing and two infant sleeping. Five children records and two caregiver record reviewed on this date.

Information gathered during the inspection determined area of non-compliance. Areas of non-complaince is noted on the violation notice.

Based on record review and interview, the licensee did not ensure parent's written acknowledgement of the receipt of required infomation.

Evidence:
Child #1 did not have written acknowledgement of Liability insurance and provisions of the family day home's emergency preparedness and response plan.

Child #2 did not have written acknowledgement provisions of the family day home's emergency preparedness and response plan.

Center had parent information but did not obtain the parents written acknowledgement.
