During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on December 16, 2024, and completed on December 16, 2024, as a part of the licensure period. The on-site inspection began at 10:10 am and ended at 2:00 pm. The inspector reviewed compliance in the areas listed above. There were 106 children present and 19 staff. The inspector reviewed ten children?s records and ten staff records on-site. This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
The record of child #9, contained documentation of immunizations dated three days after the first day of attendance.
Plan of Correction: Per the Director: We will contact the families via email to get missing information.
Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.
The record of child #7, under two years of age, did not contain documentation of additional immunizations. The most recent immunizations were documented a year ago.

Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.
There was no physical examination in the record of child #3, who had been enrolled for five months.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all required information.
