During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on May 21, 2025, and completed on May 21, 2025, as a part of the licensure period. The on-site inspection began at 12:08 pm and ended at 3:00 pm. The inspector reviewed compliance in the areas listed above. There were 114 children present and 19 staff. The inspector reviewed ten children?s records and nine staff records on-site. This inspection included document review, a tour of the facility, interviews, observations, and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-3
The center is required to submit a request for a central registry check prior to the employee's first day of employment.
1. The record of staff #2 did not contain documentation of a request of a central registry check prior to employment.
2. The record of staff #3 contained documentation of a request of a central registry check was dated 17 days after the date of employment.
3. The record of staff #5 contained documentation of a request of a central registry check was dated 7 days after the date of employment.
4. The record of staff #6 contained documentation of a request of a central registry check was dated 19 days after the date of employment.
5. The record of staff #7 contained documentation of a request of a central registry check was dated 5 days after the date of employment.
6. The record of staff #8 contained documentation of a request of a central registry check was dated 9 days after the date of employment.
7. The record of staff #9 contained documentation of a request of a central registry check was dated 1 day after the date of employment.
Plan of Correction: Per the administration: Going forward, all background CRS request will be submitted prior to hire.
Standard 8VAC20-780-240-A
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment. The record of staff #7, employed for 94 days did not contain documentation of completion of the Virginia Department of Education-sponsored orientation.
Plan of Correction: Per the administration: Will be completed by 05/23/2025.
Standard 8VAC20-780-510-F
The medication authorization shall be available to staff during the entire time it is effective.
Child 2M had a prescription emergency medication, Albuterol Inhaler, at the center. There was no medication authorization form available to staff for emergency medication for child #7. The medication had not been administered.
Plan of Correction: Per the administration: Parent will sign short term authorization 5/21/2025, or take medication home.
Standard 8VAC20-780-70
Staff records shall be kept for each staff person and contain all required information.
The record of staff #3, employed for four months, did not contain a second reference that was checked before employment. Documentation of two or more references as to character and reputation as well as competency are required to be checked before employment or volunteering.
Plan of Correction: Per the administration: All staff will have two completed references prior to hire.
