During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on October 6, 2025, and completed on October 6, 2025, as a part of the licensure period. The on-site inspection began at 10:12 am and ended at 1:35 pm. The inspector reviewed compliance in the areas listed above. There were 112 children present and 19 staff. The inspector reviewed ten children?s records and ten staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-240-A
Repeat
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment. 1. Staff #3, employed for two years and four months, did not complete the Virginia Department of Education- sponsored orientation. 2. Staff # 5, employed for three months and 27 days, did not complete the Virginia Department of Education- sponsored orientation.
Plan of Correction: Per the administration: Faculty members will have this completed ASAP. Future faculty will have this completed within 90 days.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. The medication authorization for child #11 expired four months ago. The medication was on site.
Plan of Correction: Per the administration: Family has been notified to have authorization renewed ASAP.
Standard 8VAC20-780-530-A-1
At least one staff in each classroom or area where children are present shall have, within 90 days of employment, current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care from an organization such as the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council. The training shall include an in-person competency demonstration. Staff #5, alone with a group of children on October 6, 2025, did not have certification in CPR.
Plan of Correction: Per the administration: Staff member is scheduled for CPR/FA class on 10/14/2025. Another facility member will be present in the class.

At least one staff in each classroom or area where children are present shall have, within 90 days of employment, current certification in first aid from an organization such as the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council. Staff #5, alone with a group of children on October 6, 2025, did not have certification in first aid.
