Standard 8VAC20-780-150-B
Each report of immunizations shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department. The record of child #3, child #6, and child #7, contained documentation of immunizations that were not signed by a physician.
Plan of Correction: Per the director: Management will ensure all documents are accurate and up to date, prior to child's enrollment.
Standard 8VAC20-780-160-A
Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician designee, or an official of the local health department. The record of staff #5 contained a tuberculosis screening dated two months prior to employment.
Plan of Correction: Per the director: Management will ensure TB's properly administered within 30 days before hire date.
Standard 8VAC20-780-260-A
The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. The most recent fire inspection was exceeded the annual requirement by one month.

Standard 8VAC20-780-60-A-8
The center shall maintain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. The record of child #17, with a confirmed diagnosed food allergy, did not contain an allergy care plan.
