During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced inspection was initiated and completed on November 21, 2024 in response to a complaint received by the licensing office on November 20, 2024 relating to allegations related to children's records, injury reporting and supervision. This inspection included observations and interviews. The preponderance of evidence gathered during the investigation supports the allegations related to supervision; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 19, 2024. A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION/SYSTEMIC DEFICIENCY

The staff-to-children ratios for children ages birth up to 16 months are required whenever children are in care.

On November 21, 2024 at approximately 9:01 a.m., the younger infants classroom had 9 infants present with 2 staff, exceeding the 1:4 ratio. At approximately 9:11 a.m., the older infants classroom had 6 infants present with 1 staff exceeding the 1:4 ratio. At approximately 9:27 a.m., the Toddlers classroom had 10 children present, including Child #1 (age:15 months), with 2 staff, exceeding the 1:4 ratio.

On November 21, 2024, at approximately 8:36 a.m., the Beginners classroom (two year old classroom) had 11 children present with 1 staff, exceeding the 1:8 ratio.

The staff-to-children ratios for children ages 3 years old to school age eligible are required whenever children are in care.

On November 21, 2024, at approximately 9:02 a.m., the Intermediates classroom (three year old classroom) had 15 children present with 1 staff, exceeding the 1:10 ratio. At approximately 9:05 a.m., the Pre-K classroom (ages: 4 to 5 years) had 23 children present with 2 staff, exceeding the 1:10 ratio.
