During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on March 5, 2025. The on-site inspection began at 8:45 a.m. and ended at 10:35 a.m. The inspector reviewed compliance in the areas listed above. There were 65 children present and 11 staff. The inspector reviewed 7 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, observations and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on March 19, 2025. A POC submitted after this date will not appear on the public website.

5Staff #4's follow-up TB screening is overdue by two weeks.
Staff #5's follow-up TB screening is overdue by 17 months.
There was chipped paint on walls in four classrooms. Two of the four classrooms also had peeling paint on walls within reach of children.
The allergy lists posted in each classroom were outdated. Child #4 is listed as having a food allergy, however Staff #4 confirmed that the child has outgrown the allergy. Child #7 is listed as having a food allergy, however Staff #4 confirmed that the child does not have an allergy. Child #8 is listed as having a food allergy, however Staff #4 confirmed that the child is no longer enrolled.
At the time of inspection, there was no documentation of evacuation drills for December 2024 or January 2025.
Child #'1's record indicates multiple food allergies. Child #1 has been enrolled for over two months and does not have a written care plan on file.
