Inspector Notes

An unannounced monitoring inspection was initiated and completed on September 25, 2025 in response to a self-report received by the licensing office on September 24, 2025 relating to supervision. The inspector reviewed 1 child's record and 3 staff records on-site. This inspection included document review, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to CPR/First Aid certification and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 24, 2025. A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION

When staff are supervising children, they shall always ensure their care, protection, and guidance.

On September 23, 2025, Child #1 (age: 2 years) was found, unsupervised, in the center's lobby by Staff #1. Child #1 was supposed to be on the playground, under the supervision of Staff #2, Staff #3, and Staff #4. Staff #2, Staff #3 and Staff #4 were not aware that Child #1 had left the playground and entered the building. The center was previously cited on September 18, 2025 for not ensuring the care, protection, and guidance of children.

REPEAT VIOLATION

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On September 23, 2025, Child #1 (age: 2 years) was found, out of sight and sound supervision, in the center's lobby by Staff #1. No center staff were present at the time. The child?s teachers and class were on the playground. Staff #2, Staff #3 and Staff #4 were not aware that Child #1 had left the playground and entered the building. The center was previously cited on September 18, 2025 for a child under 10 years of age not being within actual sight and sound supervision of staff.
Plan of Correction: Retrain and review supervision and face to name policies. Monitor all transitions to ensure compliance

REPEAT VIOLATION/ SYSTEMIC DEFICIENCY

At least one staff in each classroom or area where children are present shall have current certification in cardiopulmonary resuscitation (CPR) and current certification in first aid.

On September 23, 2025, Staff #2, Staff #3 and Staff #4 worked together on the playground with a group of children. Staff #2, Staff #3 and Staff #4 do not have current certification in CPR and first aid.
