REPEAT VIOLATION
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies.
The allergy lists (dated October 7, 2025), posted in the center's classrooms, do not list one of Child #5's food allergies. Child #5's (age: 22 months) physical examinations that are dated February 27, 2025, August 22, 2025 and September 4, 2025, as well as a form completed by the child's parent on August 1, 2025, indicate the allergy. Child #5 has been enrolled for nine months.

REPEAT VIOLATION
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.
Two medications belonging to Child #6 were observed on-site during the inspection. The authorizations for both medications expired over four months ago. The medications were not disposed of by the center.
