REPEAT VIOLATION/ SYSTEMIC DEFICIENCY
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies.
Documentation in Child #2's record, provided at the time of enrollment, indicates a food allergy. At the time of inspection, Child #2 was not listed on the allergy lists (dated 01/05/26) posted in the center's classrooms. Child #2 has been enrolled for 17 days.

When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.
A medication belonging to Child #6 was observed on-site during the inspection. The authorization for the medication expired 17 months ago. Child #6's parents did not renew the authorization and the medication was not disposed of by the center. The center was previously cited in October 17, 2024, December 22, 2025 and January 20, 2026 for not obtaining a renewed authorization or disposing the medication.

Child #1's (age: 3 years) record indicates a food allergy. Staff #1 confirmed that they were notified of the child's allergy upon enrollment. The center does not have a written care plan on file. Child #1 has been enrolled for 11 months.
