Inspector Notes
On 05/10/2023, an unannounced inspection was conducted regarding a complaint received by the licensing office on 05/08/2023. The complaint involved concerns about the administration of service. Interviews with the provider and other staff were conducted regarding the allegation. The preponderance of evidence supports the allegation and the complaint was determined valid.

Based on interviews, the provider failed to ensure that the home's services were conducive to the welfare of one child.
Evidence:
1. On 05/04/2023, Staff #1 cut the fingernails of Child #1 (22 months) without parent permission to provide this service. While Staff #1 was cutting the nails, the skin of Child #1's finger was cut and started to bleed.
2. According to Staff #1, Child #1 has a history of scratching other children in care.
3. Staff #1 stated that on 05/04/2023, "[Child #1] scratched [Child #2] right under the eye. I looked at [Child #1]'s nails and they were long so I trimmed her nails."
4. According to Staff #1, one fingernail began to bleed while the fingernails were being cut. Staff #1 stated, "It did bleed a few seconds and I pressed on it and it stopped. I put a little bit of Neosporin on [Child #1]'s pinky." One of three other staff interviewed stated, "I saw [Staff #1] put [Child #1] on her lap and [Staff#1] trimmed her nails. It hurt her a little bit and she cried a little bit. It accidentally cut the skin."
5. Staff #1 stated, "It scared her and when it was clipped, she cried a little bit. It was 20 seconds for her to calm down."
6. Child #1's parent went to pick up Child #1 after work on 05/04/2023. The parent stated, "I said, 'Oh my God' look how short they are. I asked her if [Staff #1] did this to her and she nodded and said 'Mama, boo boo ouch'."
