Inspector Notes

A monitoring inspection was initiated and concluded on 10/05/2022. There were a total of 68 children with 14 staff members present. Children were observed during resting. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 6 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on children records review, the center did not ensure each child have a physical examination before the child's first attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #4 (date of enrollment 1/04/2021) did not have a physical examination in the record available for review during the inspection.
Plan of Correction: We will contact the parent form and obtain a completed physical form.
