Inspector Notes

A monitoring inspection was conduced on February 17, 2023. There were a total of 63 children with 17 staff members present. Children were observed during resting for naptime.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 8 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on staff records review, the applicant did not authorize the center to obtain a copy of the results of a criminal history record check and sex offender registry check from any state in which the agent has resided in the preceding five years by the deadline of December 31, 2020.
Evidence:

Based on staff records review, the center did not obtain documentation for each staff member at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors obtain and submit the results of a follow-up tuberculosis screening as stated in subsection B of this section.
