Inspector Notes
An unannounced monitoring inspection was conducted on 5/16/2024 at 10:40am. There were 81 (infants to 5yrs) directly supervised by 14 staff. The physical plant, 3 staff records, 7 children?s records, 7 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, eating lunch. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Violations
4Evidence: reviewed 3 staff records and found that Staff 3 was working in the capacity of a program leader, but did not have documentation of program leader qualifications.
Evidence: observed in the Young 4's classroom an unlocked cabinet with Lysol spray and disinfectant spray.
Evidence: the first aid kit only had 1 triangular bandage.
Evidence: the emergency drill log only contained the date, time, # of kids, # of staff, weather and time taken.
