Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a in-person tour of the program A monitoring inspection was initiated on 3/25/22 and concluded on 3/30/22. The provider was contacted by telephone to initiate the inspection. There were 62 children present and 11 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

1. The most recent annual health inspection report inspection available for viewing during the inspection was dated 12/11/20 and the health permit from the local health department expired 3/31/21.
2. Staff #4 (Program Director) confirmed that the annual approval from the health department had not been received.
Evidence:
1. The emergency drill log did not have documentation to indicate that two shelter-in-place drills and a lockdown drill were completed during 2021.
2. Staff #4 (Program Director) stated that two shelter-in-place drills and a lockdown drill were completed during 2021., but that she had forgot to document it on the emergency drill log.
