During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 12/16/24. The on-site inspection began at 3:30pm and ended at 5:10pm. The inspector reviewed compliance in the areas listed above. There were 70 children present and eight staff. The inspector reviewed seven children?s records on-site and three staff records. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 1/6/24. A POC submitted after this date will not appear on the public website.
Violations
5Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
In the game zone, where school age children were observed, there was an unlocked cabinet that contained disinfectant wipes, nail polish remover, and disinfectant spray. All the mentioned substances had a manufacturer?s label that stated, ?Keep Out of Reach of Children.?
During the inspection, a staff member was observed working alone with 28 children. The required ratio for the group was 1:18. The staff member was observed working over the required ratio for approximately seven minutes.
During the inspection, a first aid kit was observed on a window ledge in the computer lab. The first aid kit was accessible to the children in care.
On the day of the inspection, cheese flavored crackers and juice were listed on the menu; however, the children were observed eating whole grain chips. The menu did not have the substitution (whole grain chips) listed.
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all the required information.
1.The record for Child #1 was missing the child?s first date of attendance
2.The record for Child #2 (enrolled for 11 months) was missing the name, home address, and home phone number of one parent and the name and phone number of one parent?s place of employment.
