An unannounced monitoring inspection was conducted with the director on May 11, 2023, beginning at 11:00am and ending at 1:48pm. There were 96 children, ages 2 months through 5 years old, in the care of 11 direct care staff. The children were observed during nap time. Five children?s records were reviewed. Five staff records were reviewed. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. Five medications were reviewed and in compliance. The menu, daily attendance and emergency drill log were reviewed. The practice emergency drill log, health inspection, and fire inspection were reviewed and in compliance.

Based on observation, the facility did not ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevents access by children. Evidence: In the PreK classroom, there was an unlocked cabinet that had a bottle of Lysol, a bottle of TB disinfectant cleaning spray, a bottle of bathroom cleaner, a bottle of Fabuloso, and two bottles of disinfectant spray.

Based on observation, the facility did not ensure that all cribs, cots, mats, and beds used by children other than infants during designated rest periods had linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges. Evidence: In the transitional kindergarten classroom, there were two children that did not have bottom sheets on their cot/mats.

Based on observation, the facility staff did not ensure the diapering surface was used only for diapering or cleaning children. Evidence: In 2A's classroom, there was a pitcher and pack of disposable plates in the diapering area.
