Inspector Notes
A complaint investigation was conducted regarding an allegation of behavioral guidance, forbidden action, and failure to report an injury. The investigation was initiated with the assistant director beginning on May 12, 2023, and concluded on June 8, 2023. Staff interviews were conducted at the facility on May 15, 2019, beginning at 11:00 and ending at 12:37pm. Five staff interviews were conducted. Additional interviews were conducted May 29, 2023. Two staff records and one child's record were reviewed. Incident/Accident and written reports were reviewed. The allegations of behavioral guidance, forbidden action, and failure to report an injury were unable to be substantiated due to insufficient corroborating evidence.
