Inspector Notes
An unannounced self-report and complaint inspection was conducted on June 7, 2023, in response to a self-report that was received on May 22, 2023, and a complaint that was received by the department on May 30, 2023. The investigation was initiated on June 1, 2023, and completed on July 10, 2023. The complaint was regarding behavioral management, forbidden actions, programs, and staff qualification and training. The inspector was on site on June 7, 2023, from 9:15 AM to approximately 12:45 PM conducting interviews, completing observations, and reviewing documentation such as staff orientation training and annual training.
The preponderance of the evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.
Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the Violation Notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Violations
2Evidence:
1. Staff interviewed identified persistent behavioral problems with child #1.
2. Staff interviewed reported a notebook was used to record information in reference to the child's persistent behavioral problems and given to the guardian daily or when the notebook was at school.
3. Staff #3 reported there was no documentation available in reference to the identified behavioral problems of child #1.
Evidence:
1. Staff #1 stated they provide daily feedback to parents but do not offer formal scheduled meetings, and do not complete semiannual written information to parents.
2. The record for child #1 did not have documentation of at least semiannually in writing information on their child's development, behavior, adjustment, and needs; at least semiannual documented scheduled opportunities for parents to provide feedback on their children and the center's program.
