Repeat Violation:
Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance.

Child #2, enrolled for over three months, did not have a physical examination on record.

Each staff member shall submit documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children.

Staff #1 had a TB screen dated two days after date of employment.

Repeat Violation and Systemic Deficiency:

Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care.

Staff #1, employed for 17 months, had 11.5 hours out of 16 hours of training documented in 2025 at the time of inspection.
Staff #3, employed for over three years, had 13 hours out of 16 hours of training documented in 2025 at the time of inspection.
Staff #4, employed for over 20 years, had 12 hours out of 16 hours documented in 2025 at the time of inspection.

There shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.

At the time of inspection, there were no staff present at the center that had a current training in performing daily health observation of children.

The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.

The most recent fire inspection report was overdue by three months.

The center shall have annual approval from the health department for meeting requirements for water supply, sewage disposal system and food service, if applicable.

The most recent health inspection report was overdue by two months.

Repeat Violation:
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

In the gym where children were playing, the siding of the wall was detached exposing glue. Two light fixtures located on the ceiling in the gym had guards that were detached and were hanging from the light fixtures. The ceiling in the gym was cracked and peeling paint exposing drywall.

Children's hands shall be washed with soap and running water after toileting and any contact with blood, feces, or urine. Staff shall wash hands with soap and running water before and after a diaper change.

In classroom #2, staff #1 did not wash hands after changing a child?s diaper. Staff #1 did not wash hands of the child after a diaper change.

Over-the-counter skin products shall not be kept or used beyond the expiration date of the product.

One diaper ointment used for a child in the infant room expired 3 months ago. Another diaper ointment used for another child in the infant room expired six days ago.

Prepared infant formula shall be refrigerated, dated and labeled with the child's name.

Two children?s bottles in the infant room were not dated.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all required information.

Child # 1, enrolled for four months, did not have documentation of viewing proof of the child?s identity and age.
