During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on 03/03/2026 and completed on 03/18/2026 in response to a complaint received by the licensing office on 03/02/2026 relating to allegations of supervision. The inspector reviewed one child?s record and one staff record on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
Violations have been cited related to noncompliance resulting in an injury to a child while under the supervision of the facility/provider that required outside medical treatment. Please see the violation notice for more details.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 03/31/2026. A POC submitted after this date will not appear on the public website.
Violations
6The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child #1, who had been enrolled for 11 months, did not have documentation of an immunization record on file.
Each child shall have a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance.
There was no physical examination in the record of child #1, who had been enrolled for 11 months.
When staff are supervising children, they shall always ensure their care, protection, and guidance.
On 03/02/2026, staff #1 placed child #1, age 15 months, on the changing table for a diaper change. Staff #1 turned around to grab gloves from another staff, leaving child #1 on the changing table without protection from falling. Child #1 rolled off the changing table and fell to the floor. Child #1 sustained injuries and was taken for medical evaluation after the injury.
Staff shall ensure the immediate safety of a child during diapering.
On 03/02/2026, staff #1 placed child #1, age 15 months, on the changing table for a diaper change. Staff #1 removed their hand from the child and turned their back to the child to retrieve gloves. Staff #1 stated they knew that child #1 could actively roll. Child #1 rolled off the changing table that measured 34 inches high, and fell to the floor.
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and shall include all required information.
The injury report for child #1 was missing staff and parent signatures or two staff signatures and any future action to prevent recurrence of the injury.
Staff #1, employed for 20 months and identified as the program lead in classroom #1, was missing documentation of education that is required by the job position.
