Inspector Notes
A renewal inspection was initiated on 11/01/23 and concluded on 11/01/23. There were 12 children present with the provider and assistant. The point total was 30. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 2 staff records were reviewed. The inspection started at 9:00am and concluded at 10:30am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

There was no landline phone service active at the time of inspection. The provider confirmed that no service was in place.
Evidence:
One of the rabies vaccinations is past due, documentation shows it had expired as of 10/18/23.
Evidence:
There was no enrollment application or document for child #3 as required. The start date listed for the child was 8/17/23. The child was in care during this inspection, and had been in care since August 2023.
Evidence:
Child records #1, #2, and #4 did not have proof of immunizations as required. The listed start dates were 10/10/23 and 10/14/23. All 3 children were actively receiving care.
