Inspector Notes

A renewal inspection was initiated on 11/01/23 and concluded on 11/01/23. There were 12 children present with the provider and assistant. The point total was 30. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 2 staff records were reviewed. The inspection started at 9:00am and concluded at 10:30am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-330-A
Based on discussions the provider failed to ensure that there was a landline phone available and operational as required.

Evidence:
There was no landline phone service active at the time of inspection. The provider confirmed that no service was in place.
Plan of Correction: I will continue to work on getting landline service as required.
Standard 8VAC20-800-460-C
Based on review of animal records the provider failed to ensure that rabies vaccinations were up to date as required.

Evidence:
One of the rabies vaccinations is past due, documentation shows it had expired as of 10/18/23.
Plan of Correction: I have already scheduled for the updated rabies shot as required.
Standard 8VAC20-800-60-B
Based on review of child records the provider failed to ensure that child records contain required enrollment information.

Evidence:
There was no enrollment application or document for child #3 as required. The start date listed for the child was 8/17/23. The child was in care during this inspection, and had been in care since August 2023.
Plan of Correction: Another enrollment form will be given to the parent for completion before the child is allowed to attend again. I will review children records for accuracy more frequently.
Standard 8VAC20-800-90-A
Based on review of child records the provider failed to ensure that proof of immunizations were obtained before the first day of care as required.

Evidence:
Child records #1, #2, and #4 did not have proof of immunizations as required. The listed start dates were 10/10/23 and 10/14/23. All 3 children were actively receiving care.
Plan of Correction: Proof of immunizations will be obtained and placed in the files.
