Inspector Notes

An unannounced, on-site renewal inspection was initiated on 11/04/24 and completed on 11/04/24. The on-site inspection began at 11:40am and ended at 12:55pm. The inspector reviewed compliance in the areas listed above. There were 9 children present and 2 caregivers; the point total was 25. The inspector reviewed 5 children?s records and 2 caregiver/household member(s) records on-site. This inspection included document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening.

Caregiver #1 had an expired TB screening on file. The screening on file had been expired for over 3 weeks.
Plan of Correction: A new screening will be obtained and placed in the file.
Standard 8VAC20-800-330-A
A landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation. An operable landline telephone is one that does not require electricity to operate. Cordless or cell phones may be used in addition to the landline telephone.

(REPEAT VIOLATION)
There is no working landline phone in the home as required.
Plan of Correction: I am working on securing a landline as required.
Standard 8VAC20-800-80-A
Within seven business days of the child's first day of attendance at the family day home, the provider shall obtain from the parent proof of birth documentation.

(REPEAT VIOLATION)
Child #1 has been enrolled and attending over 3 weeks and there is no verification of proof of birth as required.
Plan of Correction: Proof of birth will be verified and documented.
Standard 8VAC20-800-800-A
The family day home shall have a written emergency preparedness and response plan.

The emergency preparedness and response plan could not be located during the inspection.
Plan of Correction: The plan will be located and placed back in the file. If it can't be located a new one will be completed.
Standard 8VAC20-800-90-A
Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized.

Child #1 has been enrolled and attending for over 3 weeks and does not have proof of being adequately immunized as required.
