The department received a complaint on 1/07/2022, regarding allegations in the area of physical plant. A complaint inspection was initiated on 1/25/2022 and concluded on 1/25/2022. The inspector was onsite on 1/25/2022 from 1:46pm to 2:06pm. The inspector toured the facility.
The evidence gathered during the investigation did not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there were violations found during the inspection regarding physical plant. See the violation notice for more details.

Based on observation, the center did not ensure areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
