Violations

1
Standard 8VAC20-780-60-A-8
Based on a review of children's medication records, it was determined that the school did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician.
Evidence:
The record for Child F includes documentation indicating that the child has a food allergy. There is no food allergy action plan for this child.
Plan of Correction: I will ask the parent to have the FARE form completed and signed/dated by the physician.
