Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on September 11, 2024 as a part of the licensure period. The on-site inspection began at 9:15am and ended at 10:15am. The inspector reviewed compliance in the areas listed above. There were 16 children present and 3 staff. The inspector reviewed 5 children's records and 3staff records onsite. This inspection included document review, tour of the facility and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

1Based on review of children's records, not all records contain the required information for two emergency contacts.
Evidence
The record for child #1 and #2 did not contain documentation of the name, address, and phone number of a second designated person to call in an emergency if a parent cannot be reached.
