Inspector Notes

An unannounced monitoring inspection was conducted on 11/21/22 from 11:25am to 1:45pm with the Director. There were 56 children in care, ranging in age from four-months to eight-years-old, supervised by 12 staff. The children were observed getting ready for nap, playing on the playground, listening to a story, and having free play. Six child records and six staff records were reviewed. The Center has multiple staff with current certification in CPR and First Aid, and four staff trained in Daily Health Observation Training. Seven medications and corresponding authorization forms were reviewed and the center has four staff with current certification in Medication Administration Training (MAT). The attendance, menu, and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 3/10/22 and the most recent Health Inspection on file was dated 11/4/22. Standard 8VAC20-780-130-A
Based on review of six child records, the center did not obtain documentation that each child had received the immunizations required by the State Board of Health before the child attended the center. Evidence: The immunizations on record for Child B, with a start date of 2/7/22, were dated 5/20/22.
Plan of Correction: Immunizations and physicals are currently due prior to starting.
Standard 8VAC20-780-140-A
Based on review of six child records, the center did not obtain documentation of a physical examination, by or under the direction of a physician, for each child within 30 days after the first day of attendance. Evidence: The physical on record for Child B, with a start date of 2/7/22, was dated 5/20/22.
Plan of Correction: Immunizations and physicals are currently due prior to starting.

Based on interview, the center did not ensure that lockdown procedures were practiced at least annually. Evidence: The Director reported that the center has not yet practiced a lockdown drill.
Plan of Correction: We practice lockdown procedures during new hire orientations and will practice drills annually.
