Inspector Notes
An unannounced inspection was initiated on 3/7/1015 and completed on 3/28/2025 in response to a complaint received by the licensing office on 3/6/2025 relating to allegations of documentation of injuries, staff training/qualifications and behavioral guidance. The inspector reviewed 1 child record and 2 staff records on-site on 3/12/2025. This inspection include document review and classroom observations.
The preponderance of evidence gathered during the investigation supports some areas of the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/16/2025. A POC submitted after this date will not appear on the public website.
Violations
3Witness #1 stated that during the drop off routine, child #1 was greeting witness #1 while they were leaving the classroom. Child #1 was pulling towards witness #1 at the classroom door, while staff #1 held child #1 by the lower arm. Witness #1 stated that staff #1 "pulled [child #1] away from the door by the arm so bad". The redirection from staff 1 was not constructive in nature or age appropriate.
An email thread containing childrens and staff names and information regarding a classroom was forwarded from the center administration to a family outside of the center, unintentionally. The email thread was not treated confidentially.
The parents of child #1 stated that the child had received a splinter while in care at the center. Center management confirmed that child #1 had received a splinter. Center management could not locate a written record of the childs injury.
