During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 6/18/2025 and completed on 7/1/2025 in response to a self-report received by the licensing office on 6/12/2025 relating to supervision. The inspector reviewed 1 child record and 1 staff record on-site on 6/18/2025. This inspection included document review, tour of the facility, interviews, observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection and guidance.

1. On 6/11/2025, staff #1 brought a group of 8 children from the playground into the pre-k classroom so that child #1 (3 years old) could go to the bathroom. Approximately 5 minutes later, staff #1 left the classroom with 7 children to go back onto the playground, not realizing that child #1 remained in the bathroom. Approximately 3 minutes later, child #1s guardian arrived at the center to pick up the child.
2. Staff #1 did not ensure the care and protection of child #1 when the child was left unsupervised in the pre-k classroom for approximately 3 minutes.
Plan of Correction: In response to the incident, the Assistant Director and Regional Manager immediately placed the staff member involved on Administrative Leave and notified the child?s parents. A self-report was made to licensing and plans were made for retraining of all staff.

The daily playground schedule has been updated to ensure no class is outside after 4:30pm, which coincides with pick up time. Training on Transition Tracking and Supervision of Children Indoors and Outdoors will be completed on
7/11/2025. All staff will be retrained on the policies and procedures to both supervise, transition, conduct a name-to-face count and sweep an area prior to leaving.

Additionally, Center Leadership now escorts for all classes transitioning to and from the playground. The center leader must also complete a name-to-face transition track for each class as they leave the classroom to enter the playground area and when returning to their classroom. The Regional Manager will conduct random observations of all aspects of transition tracking and supervision during their monthly visit to ensure that all policies and procedures are being followed.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures.

1. The centers "Transition Tracking Policy" states that "Teachers are required to conduct a name-to-face roll call with child count at the following times: when leaving a location or entering a new location". Staff shall "Conduct a verbal head count at the following times: when passing through any additional door/gate on the way to a location. "In addition, the last teacher leaving a location is responsible for conducting a visual sweep, which consists of viewing all areas in the classroom to confirm no child is left behind".
